What is the Microbiome and Why Does It Matter?

The microbiome refers to the trillions of microorganisms living inside your body, primarily in the intestines. These microbes perform essential functions such as digesting food, synthesizing vitamins, and regulating the immune system. SIBO: Small Intestinal Bacterial Overgrowth

SIBO (Small Intestinal Bacterial Overgrowth) occurs when there is an abnormal increase in bacteria in the small intestine, an area that normally contains far fewer bacteria compared to the colon.

Main Symptoms of SIBO:

  • Abdominal distension especially after eating
  • Excessive gas and flatulence
  • Abdominal pain (cramping)
  • Diarrhea or constipation (or alternating between both)
  • Malabsorption of nutrients
  • Fatigue and general discomfort

Diagnosis is made through a hydrogen breath test, which measures gases produced by bacteria after consuming a lactulose or glucose solution.

IMO: Intestinal Methanogen Overgrowth

IMO (Intestinal Methanogen Overgrowth), formerly known as "methane SIBO," refers to an overgrowth of methanogenic archaea, organisms different from bacteria that produce methane as a metabolic byproduct.

Key Differences from SIBO:

  • In IMO, the predominant gas is methane, while in SIBO it is hydrogen
  • IMO is more frequently associated with constipation
  • Methanogens are archaea, not bacteria
  • Treatment may require specific antibiotics different from those used for SIBO
SIBO
  • Gas: Hydrogen
  • Main symptom: Diarrhea
  • Organism: Bacteria
  • Location: Small intestine
  • Diagnostic test: Hydrogen breath test
IMO
  • Gas: Methane
  • Main symptom: Constipation
  • Organism: Methanogenic archaea
  • Location: Intestine (can include colon)
  • Diagnostic test: Hydrogen and methane breath test

How Are They Properly Diagnosed?

Differential diagnosis is essential because treatments vary significantly. The breath test is the primary tool and must measure both hydrogen and methane to detect both conditions accurately.
